At Klinik Insepar in Tasikmalaya, the "Prolanis" (Chronic Disease Management Program) initiative was born out of a concern by Dr. Syarhan, the clinic's lead, regarding the influx of patients with uncontrolled asthma. Dr. Syarhan observed a tendency for asthma treatments to rely heavily on acute care, which can lead to dependency; care was episodic, reactive, and hospital-based, with an inclination towards scientifically unverified alternative therapies. Meanwhile, the existing chronic disease management programs in the region were limited to diabetes and hypertension.


To bridge this gap, Klinik Insepar developed and implemented a comprehensive asthma management program that moves away from dependency on acute medical treatments. The initiative includes continuous education for patients and their families in the local Sundanese language, aiming to enhance their ability to independently manage asthma and prevent exacerbations. A WhatsApp group for asthma patients facilitates the exchange of management tips, and the clinic organizes joint exercise sessions to improve patients' physical resilience and reduce asthma recurrences. This Prolanis program is financed by BPJS Kesehatan (Indonesian Social Health Insurance) and collaborates with local community health centers (Puskesmas), referring asthma patients to the clinic and maintaining regular data reporting to the Puskesmas.


The impact of this initiative has been significant. During the six-month pilot project (2014-2015), 416 patients were enrolled, with 60% achieving controlled asthma status. The number of patients experiencing exacerbations decreased dramatically from 20 patients per month in 2012 to 1-2 per month in 2023, mostly new cases. Furthermore, from January to October 2023, 33 patients have maintained controlled asthma status, showcasing the program's effectiveness in managing this chronic condition.
