The Puskesmas Girimulyo II in Kulon Progo has launched the initiative "SI MAS HEBATKU" (Community Synergy for a Healthy Smoke-Free Life in Kulon Progo), aiming to enforce smoke-free zones (KTR) and control risk factors for non-communicable diseases (PTM). The motivation behind this program is grounded in local legislation, specifically the Regional Regulation No. 5 of 2014 regarding Smoke-Free Areas. Despite the existence of such policies, there has been a historical low adherence to Public Healthy Living Behaviors (PHBS) within the areas served by the Puskesmas. This initiative seeks to reshape the community's mindset towards KTR from one of total smoking prohibition to one of considerate smoking behavior, such as avoiding smoking near children and pregnant women and during community events.


To uphold the KTR regulations and improve PHBS indicators, several strategies have been implemented. One key activity is the declaration of "Dusun KABAR" (Smoke-Free Area Villages), which is tailored through discussions with village heads and the community, leading to customized rules for each village based on local needs and readiness. Community health workers play a vital role in education and monitoring PHBS, as well as in displaying "Thank you for not smoking here" stickers. The initiative also includes sudden healthy clean inspections without cigarette smoke, known as "Sidak sebentar kok," conducted with the KTR task force, which involves removing cigarette advertisements and vendors’ displays, as well as conducting school inspections to prevent student smoking.


The "SI MAS HEBATKU" initiative has evolved over its seven-year journey, with recent additions including the collection of primary data on individual smoking behavior. The impacts have been tangible, with a reported reduction in household smoking and increased compliance with the KTR ordinance. This is a significant step towards improving public health and fostering a healthier environment in the community of Kulon Progo.
